When the belly isn't fat

A surprising number of postpartum patients spend months trying to diet away a lower-belly bulge that was never fat to begin with. It's diastasis recti, a separation of the two halves of the rectus abdominis muscle down the midline of the abdomen. No amount of caloric restriction closes a structural gap, which is exactly why these patients feel like they're failing when they aren't. The first step is simply finding out whether a gap is what you're dealing with.

The two-minute self-test

Lie on your back, knees bent, feet flat on the floor. Place the fingertips of one hand horizontally across your midline, just above your belly button, fingers pointing toward your toes. Relax your stomach, then slowly lift your head and shoulders slightly off the floor, as if starting a small crunch. As you lift, press your fingertips gently into your abdomen. You're feeling for a soft gap between the two firm ridges of muscle on either side. Note how many fingers fit in the gap, and check above and below the navel too.

What the width means

Width is measured in finger-widths. A gap of one to two finger-widths is generally considered normal, especially in the first months postpartum. Two to three finger-widths suggests a meaningful separation worth addressing. Three or more finger-widths is a significant diastasis. Also pay attention to depth: a gap that feels shallow with firm tension below it is in better shape than a deep gap where your fingers sink toward your spine, which signals weaker connective tissue.

Why crunches can make it worse

Here's the trap: the instinct is to do more ab work, but traditional crunches and sit-ups increase intra-abdominal pressure and can push the separation wider, sometimes producing a visible "doming" or "coning" down the midline as you lift. If you saw your belly peak into a ridge during the self-test, that's the sign. The abdominal wall needs to be rebuilt with the right kind of tension, not loaded in a way that drives the two sides further apart.
