This policy covers Bromberg Wellness (https://brombergwellness.com), a division of Bromberg Chiropractic, at 201 Broadway, First Floor, Cambridge, MA 02139. It explains exactly what this website collects, why, where it goes, and how to have it removed.
The short version. If you fill in the consultation form, we collect what you type plus your IP address, store it on our own server, and email it to the practice so someone can call you back. We use Google Analytics to count visits. We do not sell your information, and we do not use it for advertising.
The consultation form on this site asks for the following. Only your first name, last name, email address and treatment interest are required — everything else is optional.

This site does not set advertising cookies, does not run remarketing or advertising pixels, and does not have user accounts or logins.
Information from the consultation form is used only to respond to your enquiry and to arrange and provide care. We do not sell it, rent it, trade it, or use it to build advertising audiences. We do not send marketing email to addresses collected through this form unless you separately ask us to.
Each submission is written to a file on this website's own server, which is not reachable from the web, and is also delivered to the practice by email. The practice's email account is therefore the long-term record. Submissions that our spam filter blocks are kept in a separate log for the same reason — so that if a real enquiry is stopped in error, we can still find it and call you back.
We keep enquiry records for as long as needed to respond to you and to meet the practice's record-keeping obligations. You can ask us to delete yours at any time — see section 6.
We share information with the following service providers, and no one else. Each acts on our instructions and none of them is permitted to use your information for their own purposes.

We may also disclose information if we are legally required to, or to establish or defend legal claims.
The consultation form is an enquiry form. It is not a secure medical messaging system, and it is not the right place for detailed medical history, test results, or anything you consider sensitive. Please share that with us by phone at (617) 494-1166, or in person at your consultation. Information you give us once you are a patient of the practice is handled under the practice's own patient-privacy practices, which are separate from this website policy.

This site and these treatments are intended for adults. We do not knowingly collect information from anyone under 18. If you believe a child has submitted information to us, contact us and we will delete it.
If we change how this site handles your information, we will update this page and the “last updated” date above.
