Restoring the Neural Feedback Loop Between Pelvic Floor and Detrusor Muscle

Urge incontinence — the sudden, overwhelming need to urinate that sometimes results in leakage before reaching a bathroom — is mechanistically distinct from stress incontinence. While stress incontinence is a muscle strength problem, urge incontinence involves a dysfunctional neural feedback loop between the pelvic floor muscles and the detrusor muscle (the smooth muscle wall of the bladder). In healthy function, the pelvic floor muscles send proprioceptive signals to the central nervous system that help regulate detrusor activity. When the pelvic floor is engaged (contracted), it sends inhibitory signals that suppress premature detrusor contractions, allowing the bladder to fill to appropriate capacity before triggering the urge to void. When pelvic floor muscles weaken, this inhibitory feedback degrades. The detrusor muscle begins to contract prematurely and involuntarily — sometimes in response to minor stimuli like running water, cold air, or simply approaching a bathroom (the "key in the lock" phenomenon). Emsella's HIFEM protocol targets this feedback dysfunction from the muscular end of the loop. By strengthening the pelvic floor muscles and increasing their proprioceptive output, Emsella restores the inhibitory signaling that suppresses premature detrusor contractions. The supramaximal contractions also improve the pelvic floor's ability to physically override an unwanted detrusor contraction through reflex clamping — even if the detrusor fires prematurely, a stronger pelvic floor can generate enough counter-pressure to prevent leakage and buy time to reach a bathroom. Over the treatment series, patients typically experience both a reduction in the frequency and intensity of urgency episodes (from restored neural inhibition) and an improved ability to "hold it" when urgency does occur (from increased muscular strength). This addresses both the sensory and the motor components of urge incontinence.
