Menopause creates a specific and compounding challenge for pelvic floor health. Declining estrogen levels directly affect three tissue systems simultaneously: the skeletal muscle fibers of the pelvic floor lose mass and contractile strength (sarcopenia), the connective tissue (fascia and ligaments) that suspends the pelvic organs loses elasticity and structural integrity, and the mucosal tissues of the vagina and urethra thin and become less resilient.
