The Hidden Link Between Your Pelvic Floor and Lower Back Pain

If you've chased lower back pain with stretches, massage, and core workouts without lasting relief, you may be missing a muscle group nobody thinks to check. The pelvic floor is part of your core — and when it's weak, your back often pays the price.

The Hidden Link Between Your Pelvic Floor and Lower Back Pain

Your pelvic floor is part of your core — most people don't know it

When people think "core," they picture abs. But the core is not a single muscle; it's a coordinated group, and the pelvic floor is its foundation. These muscles form the bottom of a structure that stabilizes your spine and pelvis with every movement you make. Because the pelvic floor is hidden and rarely discussed, it's the piece almost everyone overlooks — including many people who diligently train everything above it and still can't resolve their back pain.

The canister model: how the core actually works

Think of your core as a canister. The diaphragm is the lid, the deep abdominal and back muscles form the walls, and the pelvic floor is the base. When all four work together, they generate the internal pressure that braces and supports your lumbar spine — the way an inflated can resists being crushed. If the base of that canister is weak, the whole system loses stability. The pressure that should support your spine leaks away, and the structures around your lower back are forced to compensate.

Why a weak pelvic floor shows up as back pain

When the pelvic floor can't hold up its end of the core, other muscles take over jobs they weren't designed for. The lower back muscles overwork to provide stability the deep system isn't delivering, and chronic overwork becomes chronic pain. This is why some back pain stubbornly resists treatments aimed only at the back itself — the source isn't where the pain is felt. Until the foundation is addressed, the compensation, and the pain, tend to return.

The link people miss after childbirth and menopause

Two of the most common times for pelvic-floor weakness to set in are after childbirth and during menopause — and both are also common times for new lower back pain. That overlap is not a coincidence. Pregnancy and delivery stretch and weaken the pelvic floor, and declining estrogen in menopause reduces the strength and tone of those same muscles. Many people attribute the resulting back pain entirely to posture, aging, or carrying a baby, never realizing a weakened foundation is part of the story.

Strengthening the floor to support the spine

Rebuilding the pelvic floor restores the base of the core, and a stable base lets the rest of the system stop compensating. The challenge is that most people cannot effectively isolate and strengthen these muscles on their own — Kegels are easy to do incorrectly and easy to abandon. Emsella addresses that directly: seated and fully clothed, it drives thousands of complete pelvic-floor contractions per session, training the foundation thoroughly in a way voluntary effort rarely achieves. Stronger from the bottom up, the core can finally do its job.

When to get assessed

If you have lower back pain alongside any signs of pelvic-floor weakness — leaking with coughing or exercise, urinary urgency, a feeling of heaviness, or weakness after childbirth or menopause — the two may be connected, and treating the back alone may never fully resolve it. A proper evaluation can determine whether your pelvic floor is part of the picture. At Bromberg Wellness, Dr. Bromberg assesses the core as the integrated system it actually is, because the most effective fix is the one that addresses the real source, not just the symptom.
